Here all of the forms that you will need in order to fully participate in the Girls AA OFSAA 2005 Basketball Championship hosted by South Grenville DHS. Instructions are also included.

Team Seeding Information Form  Fill in online (type in the data), print, and then fax the Team Seeding Information Form to Mark Lewis at (613) 925-2864
Rules of Behaviour Read this Form before filling out the Rules of Behaviour Signature Form
Rules of Behaviour Signature Form  Fill in online (type in the data), print, bring the completed form to registration. 
OFSAA Pin Order Form  Follow the instructions on the form. Send the completed form to OFSAA.  
Travel Grant Application  Follow the instructions on the form. If you are eligible the form must be sent within a month of the event. 
Photo Form  Follow instructions on the form, print and fill out form. Bring the completed form to hand in at Registration
Clothing Order Form   Follow instructions on the form, print and fill out form. Email numbers and/or fax the completed form to Mark Lewis at (613) 925-2864.
