Forms and Entries
All necessary forms and entry information have been sent to your Association Convenor and you should have received these forms upon completion of your league championship game. For your convenience, all necessary forms and entry information are available to download here (please find the correct form on the left-hand menu of choices) as PDF files ( ). You will need Adobe Reader.

Coaches must complete the Entry Form, Seeding Information, the OFSAA Rules of Behaviour Form, Banquet request form, and T-Shirt Order form and send them so that they arrive no later than November 26, 2005 at 6:00 p.m.. The entry fees are due at the time of registration.

Fees
A total of five cheques need to be written for this championship. All cheques, except the one for pictures need to be made out to "OFSAA Basketball 2005" . We are asking for separate cheques to ease the registration process and help with our accounting. Your cooperation in this matter is greatly appreciated. We look forward to a great tournament and are excited about hosting this event.

Entry Fee - Cheque # 1
Cheque in the amount $350.00 payable to “OFSAA Basketball 2005"

Banquet Fee - Cheque # 2
Cheque in the amount of $20.00 per participant to a maximum of 15 participants payable to “OFSAA Basketball 2005"

T-Shirt Fee - Cheque # 3
T-shirts are available to coaches and athletes at a cost of $20.00 per short sleeve shirt and $30.00 per long sleeve shirt. Orders must be received by November 26, 2005 at 6:00 p.m. as these shirts will not be available for purchase when you arrive. Cheques should be made out to “OFSAA Basketball 2005".

OFSAA Participation Fee - Cheque # 4
Payable to “OFSAA” - $2.00 per athlete.

Team Pictures - Cheque # 5
Please see enclosed information concerning team pictures. Cheques should be made out to “Photovisions”

All entry forms, fees and seed information are to be received no later 6:00 p.m. Saturday November 26th 2005. Please fax at 613-925-2864. If all information is faxed, then all documents and fees must be submitted upon registration.

Registration
Team Registration will take place on Wednesday November 30, 2005 at South Grenville District High School in Prescott, Ontario. South Grenville DHS is located about fifteen (15) minutes east of Borckville along Hwy. 401.

Team Photos
Team pictures and Competitors badges will be taken upon your arrival at registration. Please make arrangements at registration for your picture time.

The Playing Regulations are included in this package. Please review them.

Coaches Meeting
Coaches’ Meeting will be held Wednesday November 30, 2005 at 9:00 pm at South Grenville DHS.

Banquet
The Banquet will be held at December 1, 2005 at 6:30 p.m. at the Brockville Memorial Centre. The Memorial Centre is located just North of Parkedale Avenue on Laurier Blvd. All teams are expected to attend. Please dress appropriately - no jeans allowed. More information is included here.

Souvenirs
Souvenir clothing will be available for sale throughout the tournament at South Grenville D.H.S.

Games and Championships
Games start on Thursday December 1, 2005 at 9:00 am. The Consolation Final is at South Grenville District High School on Saturday, December 3 at 2:00pm. The Bronze Medal Game is at South Grenville District High School on Saturday, December 3 at 6:00pm. The OFSAA "AA" Gold Medal Game is at South Grenville District High School on Saturday, December 3 at 8:00pm.
